Always and Forever Video and Photography (hereinafter referred to as (“Photographer”) and ______________________  (hereinafter referred to as (“Customer”) hereby agree to the following terms and conditions with respect to Photographer’s services at the following:

Photographer will make every reasonable effort to include among the Video taken any specific shots requested by Customer, provided sufficient time is permitted in the schedule of events and indicated subjects are available.  Photographer will also comply with the policy of the church or the presiding clergy with respect to photography during services.

 

The total cost of Videography agreed upon for this event is $799.00 + .06% MD State Sales Tax $47.94 = $846.94  In return, Photographer will provide Video of the scenes and events of the day and the customer will receive one Edited Video w/Music Video Segment on DVD.                                                                                

 

Customer agrees to pay Photographer a non-refundable initial deposit of $446.94 at the time this agreement is signed, to reserve the date in Photographers schedule.  A payment in the amount of $200.00 will be paid to Photographer by __/__/__ (45 days before event) and final payment of $200.00 by __/__/__ (15 days before event)

Make check payable to The Credit Card Store. Fax contract and check to 301 829 6735.  If you wish to pay by credit card then on a separate piece of paper put your name, credit card number, expiration date, and card holders billing address and fax with contract. 

If the event is canceled or rescheduled to a day that Photographer can not attend, then a refund equal to all payments received after deposit will be given as long as this does not occur less than 30 days before the event.

 

Although the utmost care will be taken with all photos, Customer agrees that Photographer’s liability for loss or damage to, any photos due to any cause,  is limited to a refund of all deposits and payments received from Customer; a reasonable pro-rata refund shall be made in the event of partial loss or damage.  Photographer’s reasonable control shall likewise be limited to a return of all deposits and payments received from Customer.

 

Customer agrees that Photographer may reproduce and use any photos taken under this agreement for display to prospective customers or for other promotional purposes.
